If you have HMO medical insurance through the Village, your vision discount provider is: EyeMed

Participants in the HMO insurance plan can access vision discounts by visiting the EyeMed website - eyemedexchange.com/blue365. Use their Provider Search tool to find a provider or call the EyeMed Customer Care Center at 866-273-0813.

If you have PPO Medical Insurance through the Village, your vision discount provider is: Davis Vision

Participants in the PPO insurance plan can access vision discounts by visiting the Davis Vision website - davisvision.com and enter client code 4513 in the Client Code field. User their Provider Search Tool or call Davis Vision at 888-897-9350.
