The Village of Hoffman Estates will once again provide wellness screenings for employees and spouses participating in one of the Village health insurance plans. These screenings consist of a comprehensive wellness blood panel and an assessment tool that evaluates your current health status and lifestyle. A participation gift card will be given out in the Spring to eligible, participating employees only.
Eligible participants can register for additional new advanced cardiac risk tests during their wellness screening. These tests are not eligible for BCBS coverage and are offered on a self-pay basis only. The associated fees would be collected on the day of the screening. How to Participate
1. Log into (or create) your Empower Health Account.

2. Within your Empower dashboard, click "Register" and follow the steps below:
Helpful tip! Fast 8 hours and drink 2 glasses of water 1-2 hours prior to your appointment.

| 3. After registering, log into your Empower dashboard and complete the Empower Health Assessment. |
| 4. Participants will receive a personalized report within 2-3 days of the event through the Empower portal. |
Our goal is to have at least 75% of Village-insured employees participate in this year's program! Therefore, we need at least 235 insured employees to participate. Last year, the Village had a total of 217 employees participate in the program, so this goal is very reachable!
The Village is very committed to this program and the more of you that participate, the higher the value of the gift card! Employees that successfully participate in the Empower wellness program will receive a gift card after the program concludes in the Spring.
- 175 employee participants - receive a $150 gift card
- 200 employee participants - receive a $200 gift card
- 235 employee participants - receive a $250 gift card
- 250 employee participants - receive a $300 gift card
Reminder! Only employees, and their spouses, currently enrolled in one of the Village's health insurance plans are eligible to participate in the Empower wellness screening program. The participation gift cards will only be provided to eligible, participating employees (not spouses) who are active at the time of distribution after the program concludes in the Spring.

Employees and spouses participating in one of the Village's BCBSIL PPO plans are eligible to participate in the annual wellness screening program.
The participation gift cards are provided only to employees, not spouses, who are actively employed at the Village of Hoffman Estates at the time of distribution after the program concludes in the Spring.
Yes! You have the option to complete your wellness screening by visiting your healthcare provider or visiting a lab. Follow the steps listed above or within the "Offsite Screening Program" flyer linked above.
