Calling 9-1-1: Gather your critical information
Nearly 75 percent of emergency calls answered by the Hoffman Estates Fire Department were medical in nature in 2025. Emergency Medical Services (EMS) continue to be at the forefront of what the Hoffman Estates Fire Department proudly provides to the community.
In the event that you need to call 9-1-1 for a medical emergency, it is imperative that the first responders who arrive at your home gather critical information about the patient to provide the best care possible, transfer the information to the Emergency Room staff, and complete an accurate medical report.
Having this information written and accessible is critical, allowing emergency medical care to be performed in a timely and efficient manner. The information that our EMS responders need to know includes the following:
- Name
- Date of Birth
- Phone Number
- Address
- Emergency Contact
- Medical History
- Medications
- Allergies to Medicines
- Advanced Directives, POLST/ DNR, Power of Attorney
